If it is a new meter set or road bore please bring the following: photo id,  a copy of the deed,  E911 address letter (662-869-2911), notice of intent form/perk test from Jackson (1-855-220-0192 or http://healthyms.com/wwapply), and if you do not have road frontage please provide a copy of easement agreement permission.
